Tamar Hummel and Dr. Leyla Sherbaz admit young patient Gina Hachmann, who fractured her hand in a fall. During initial treatment, the doctors learn that Gina has been suffering from painful hands and various other health problems for some time. Gina's mother, Sandra, assumes that her daughter is imagining the pain or even faking it to get attention. Sandra is also very busy with Gina's siblings. The relationship between mother and daughter is not in the best shape in light of the serious accusation. Tamar believes the patient and eagerly sets about finding out what's wrong with Gina... The day couldn't be going better for Ivo Maric: He's been assigned to see Dr. Matteo Moreau, with whom he wants to impress. But then Gerd Beck arrives at the Johannes Thal Clinic, unable to keep his hands still and having to repair everything. Unfortunately, Gerd slipped with the screwdriver and hit a femoral artery. But no illness and no doctor can stop him from repairing it. The doctors are annoyed, but when Matteo's 3D printer suddenly starts acting up, they rely on Gerd's talent. Is that a good idea? Julia is about to take her additional training in fetal surgery, but her study schedule has fallen victim to construction work at home. So she ends up studying in Wolfgang's office, where files are to be picked up for destruction today. Actually, that's not a problem, as long as Julia keeps a close eye on her notes.
Dr. Matteo Moreau, who was actually on his way home, found Sunny Schneider unresponsive outside the Johannes Thal Hospital and brought her to Dr. Mikko Rantala in the emergency room. The suspicion of fulminant, acute liver failure due to a blood clotting disorder was confirmed. Sunny's boyfriend, Tobias Holler, arrived almost at the same time as Sunny and was standing at her bedside in the ICU, very worried, when Sunny's father, Kai Schneider, suddenly appeared. Tobias couldn't believe his eyes: Kai was his father too. While Dr. Marc Lindner was telling his good friend Dr. Julia Berger about a promising match from a blind dating app, librarian Mario Andrea "stumbled" into the Johannes Thal Hospital with a torn hip and a joint blockage. When he saw Julia, his attention was completely focused on her, as he immediately took a liking to the doctor. Julia, too, was taken with the new patient. It seems as if Marc and Julia's love life is about to revive, but neither of them suspects where their new flirtations will lead... Matteo refuses to take a vacation. Wolfgang then challenges Leyla to demonstrate her leadership skills and resolve the matter. Leyla is firmly convinced she has her boyfriend under control, but can she really convince Dr. Matteo Moreau to devote herself to his free time?
In the emergency room, Dr. Elly Winter and Sofia Galura admit patient Nadine Förster. She was in a serious car accident, resulting in a torn heart muscle. Urgent surgery is required. At that moment, her partner, Sascha Pasing, suddenly appears in the trauma unit. Elly is startled when she recognizes Sascha. After all, he is part of her current personal problem. In a complicated operation and with the assistance of Dr. Leyla Sherbaz, Elly manages to stitch the torn heart muscle. Much to Sofia's dismay, however, after the operation, she decides to hand the case over to Leyla. Sofia and Nadine wonder what happened between Elly and Sascha. Dr. Mikko Rantala is full of anticipation, as he has a plan for youth development and is expecting a student intern. Maja Kieninger is in 11th grade and, at her parents' request, is to complete a two-week internship at the Johannes Thal Hospital. Mikko and the resident doctor, Oliver Probst, quickly realize that Maja, with her clumsy manner, isn't exactly a natural when it comes to medicine. When she then cuts up Mikko's work clothes, he has to take a deep breath. Ivo had a female visitor. Now Tamar wants to find out if Sofia has a problem with that. The young doctors negotiate several times about the strange visitor, but they all agree on one thing: Ivo and Sofia aren't a good match anyway. But when the evidence mounts that Sofia is head over heels in love with Ivo, the friends are speechless. What now?
Young adults Joshua Tremmel and Luca Münch bonded over their cancer and have been inseparable ever since. They live in a shared apartment and go through every treatment together. Christel Lacell comes to the Johannes Thal Hospital because she broke her wrist. Due to her slight tremor, she couldn't hold on and fell awkwardly. Her partner, Alfons Nystedt, is worried about her. When he wants to visit Christel after the surgery, he is irritated and hurt that Christel sends him away and doesn't explain why. When Sofia Galura and Dr. Leyla Sherbaz learn the reason for Christel's behavior, they are perplexed. Since Julia and Elly have done a fantastic job as interim instructors, Olli and Tamar consider how they can return the favor. It's not easy to please them. Ben quickly gives the interns an idea - and even benefits from it himself.

She suddenly collapses on the operating table with a devastating headache and meningismus. Her friends Dr. Ben Ahlbeck and Dr. Julia Berger immediately attend to her. Meanwhile, her husband Beat Leuenburger has a bad feeling and comes to the hospital. Dr. Matteo Moreau immediately enlists support to help Elly. A long period of anxiety begins for Beat and Elly's friends. But when Elly comes to, everything seems normal at first. At first... Interns Tamar Hummel and Sofia Galura have been preparing for Karlotta Krämer's thyroid surgery with Dr. Elly Winter. When Elly is suddenly incapacitated, the two are faced with a strange problem: Karlotta has smuggled her turtle, Mechthild, into the Johannes Thal Hospital. Who else is going to look after the animal? Although it's strictly forbidden, Sofia and Tamar cover for her - until Mechthild disappears without a trace. And now what?. When Mikko and Florian learn about Elly's condition, they offer Karin the opportunity to take over Elly's patient. Karin is grateful but ensures that the two receive support. Despite all the efforts of her young colleagues, Karin realizes that Mikko and Florian have taken on a bit too much with their plan.
The spin-off to the original series 'In aller Freundschaft' focuses on the young doctors in a fictional clinic in Erfurt.
