Season 10 kicks off with a new theme of "paying it forward" and 21 contestant hopefuls vying for a spot at the ranch. With obesity now one of the leading causes of preventable death in the United States, "The Biggest Loser" wanted to broaden their reach this season, pay it forward and motivate communities - not just contestants - to get healthy. Trainers Bob Harper and Jillian Michaels and host Alison Sweeney travel to seven cities in the premiere episode, with each trainer leading community fitness challenges like a one-mile race or a 500 step-up competition. In each of the seven cities - Detroit, Los Angeles, Portland, Atlanta, Phoenix, Oklahoma City and Boston - there will also be three contestant hopefuls competing in the challenges. The two candidates who complete their fitness challenge first win a spot at the ranch. Those who don't make it will get a chance to vie for a spot on the show in a later episode.
The fourteen contestants learn that they will be joined on the ranch by two of the seven eliminated contestants, those two being "saved", one each by Bob and Jillian. They also learn that despite wearing a same colored t-shirt as the other contestant that made it through from their city, each of the sixteen contestants is competing as an individual. The sixteen go through their first workout with Bob and Jillian, and have their first assessment by Dr. Huizenga. Both processes are eye opening experiences, and not in good ways for most. Bob and Jillian also take the contestants into the kitchen to discuss their diet, past and future. Aaron, Brendan and Mark receive presents from home based on the discussions in the kitchen. Alison drops two bombshells on the contestants regarding their first weigh-in on the ranch: it will done earlier than they expected, and the yellow line will not be above the bottom two contestants but rather the bottom half of all the contestants. Although most contestants end up posting big weight loss numbers, the euphoria quickly turns to dismay for some as one by one a contestant falls below the yellow line. After the weigh-in, Alison tells those below the yellow line that six will be safe, one by decision, the other five in a race for their on-ranch life.
With Allie being voted off the ranch at the last weigh-in, there are fifteen contestants remaining at the ranch. At that last weigh-in, seven of the eight women were below the yellow line, and as such Jillian feels the women have to get out of what she calls their "kum-ba-yah" mentality. That overwhelming atmosphere of bonding and friendship amongst all sixteen contestants has now disappeared with the first elimination, which makes them all truly realize that they are in a competition, however a competition in which two of the remaining fifteen express not wanting to participate, which raises the ire in the remaining thirteen and in Bob. Because of the twists demonstrated in the past week, the trainers are now treating every workout as a last chance workout as no one is aware when the weigh-in will happen. One of those workouts is provided by tennis player Anna Kournikova. The contestants participate in an immunity reward challenge inspired by Anna's visit. That challenge piques the interest of most as it seems as if an alliance of three has formed. When the weigh-in does occur, the fourteen non-immune contestants learn that once again half, or seven in total, will fall below the yellow line, the biggest loser of the week will save one of the those seven, with the remaining six fighting for four spots via an elimination challenge where they must literally grab for the brass ring.
With Tina voted off the ranch after the last weigh-in, there are fourteen remaining contestants, who learn that there will be two lines this week, a half group yellow line, and a one person red line, that person being the contestant with the lowest percentage of weight loss and who will immediately be leaving the ranch. These two lines mean that two people will be going home this week. This news makes the reward temptation challenge all the more important for those that either feel the need for the reward based on low percentage of weight loss in previous weeks, or want the perceived power. The reward is a one pound advantage coupon that grows by one pound each week that it is not used. As the temptation was cupcakes, chef Curtis Stone teaches the contestants how to make low calorie cupcakes. The contestants are once again surprised by a mid-week weigh-in. After the weigh-in, Jillian takes aside one contestant who hasn't yet felt an emotional breakthrough despite having consistently good weight loss at the ranch. And Bob takes the below the yellow line contestants on a special outing, before four or five of the six of them compete in the elimination challenge (with one saved and one excluded for a medical issue, those two who could be the same person), where they have to roll out the carpet. Alison has one more surprise for the remaining twelve contestants after the vote.
With Sophie being below the red line and Burgandy the person voted off of those below the yellow line after the last weigh-in, there are twelve contestants left on campus... or so those twelve believe. However, the five contestants who never made it onto the ranch arrive, with the one of those five with the highest percentage of weight loss coming onto campus to compete with the twelve. That person learns that he/she will wield much power as that person will also determine the newly formed teams: six on the black team who will train with Jillian, and six on the blue team who will train with Bob, with the final person, who will have immunity for the week, being allocated to the team who loses a contestant this week. That person makes a surprise decision which may be their downfall if the plotting on the ranch works the way those contestants determine. The game playing irks Bob and Jillian, who get wind of the plot. Beyond their first workout, their first team task is a reward challenge, where, as a team, they must race through downtown Los Angeles the fastest, with a different team member answering a food related question at each of six stops, with wrong answers resulting in that team member needing to complete a workout time penalty. The challenge ends with surprising and selfless results. This week's weigh-in ends up being an emotional one based on the thoughts of game play.
